Community Health Assessment (CHA) 

The Community Health Assessment Stakeholder Committee provides direction and input into the ongoing Del Norte County Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP).

The CHA is the first step towards pursuing accreditation status for our local health department. It is our hope that the results of the CHA will produce a common vision and shared strategies to serve as a foundation for us to collectively and comprehensively impact the long-term health and wellness of our community.

This is the first time Del Norte County Public Health is conducting its own Community Health Assessment, looking at the health needs and resources of the community through the eyes of community members to prioritize future interventions. 


The CHA is kicking off with several Community Meetings. FREE dinner and childcare will be provided to those who RSVP:
