MCAH Nurse Home Visiting
For Women, Children, Fathers, and Families!

    Antenatal & Postnatal Nurse Visits: We visit pregnant women & new mothers in their homes to provide support during a time of incredible change. We answer health questions, listen to concerns, help with breastfeeding, help to schedule OB appointments, provide birth control counseling, aid in transportation to medical appointments & connect mothers to community resources!

    Welcome Every Baby: In Partnership with First 5 and Sutter Coast Hospital, nurses from public health visit mothers in the hospital & welcome newborns with a resource kit. We connect with mothers after they deliver, then after they are discharged to ask how mothers & new babies are settling in, answer questions, provide nurse visits to the home to weigh & measure new babies & connect mothers and families to the resources they need to raise a baby.



Children & Youth With Special Health Care Needs: We coordinate with families and care and service providers to ensure that children and youth are connected to appropriate care that meets their individual needs.
