Birth, Death and Marriage Records
There are two ways you can receive a vital record copy request, they are as follows:
If you would like to obtain your Birth, Death or Marriage Certificate by coming into our office location, you will need to complete an application prior to your visit or at the time of your arrival to our office. Payment will be collected at the point of sale in the office (not online).

BY MAIL – With a check, money order or cashier’s check. Do not send cash through the mail.
If you want to purchase a Birth, Death or Marriage Certificate by mail:

- Authorized Certified Copies for Vital Records require the Sworn Statement MUST be signed in the presence of a Notary Public. Please note only Authorized Individuals may receive a certified copy. Unauthorized individuals may receive informational certified copies of birth, death and public marriage certificates.
- All of these forms must be mailed to the Del Norte County Clerk-Recorder’s office along with the proper fees and a self-addressed stamped envelope. See acceptable payment methods above.
